The DHS Athletic Department will begin issuing gold cards for the 2017-2018 school year on July 5, 2017.

Every year, each athlete is required to view an Activities Code of Conduct Meeting and get an up-to-date physical for the next season in which they plan to participate. Below is the Link to the Presentation & a PDF of the forms a parent/guardian and student must fill out and return to the athletic office.

Once the student brings in the Code Meeting forms (Insurance Waiver, Emergency Form, Code Acknowledgement, Midwest Orthopedic Consent form, IHSA Drug Testing Policy form, & Concussion form) along with an up-to-date physical & the $100 activity fee, the DHS Athletic Office will present the athlete with a Gold Card.

This card will be the athlete’s “Golden ticket” into his/her first day of practice or tryouts. He/She will not be able to participate until the coaches have this card in their hands.

In the event that the student loses this Gold Card after it has been issued to him/her in the office, a $5 charge will be required to receive a replacement. THIS CARD IS VERY IMPORTANT to the athlete’s participation. We highly encourage the students take responsibility for their involvement in their particular sport, and that starts with making sure that all the necessary paperwork is submitted in a timely fashion.
